attacking from behind while not holding block usually works 
you can strafe behind things easily to get them too. Your PvE becomes a lot more effective if you can BS

The trick is to not be pressing any other button when you backstab. If you're blocking, you'll just do a normal attack. If you're walking, you'll do a moving attack. Get behind the guy, stop moving for a second, and pound the attack button.

Spot on. Another tip is to circle around his weapon hand and not the off hand. Once behind the weapon you can turn around and get the BS animation. I've been using this tactic a lot against the spear silver knights in Anor.

I find the best way is to run at your target. Run straight for them, once you're within arm's length turn just enough to brush their side and, as soon as you're past them, quickly pull down on the analog stick and attack. Instant backstab! You can do this so quickly enemies won't even have time to swing their weapon.
