You just have to kill all of the chests that eat you, the one in Sens, the three in Anor Londo, and then the rest are in dukes.

Killing all of the chest's does not guarantee the drop.
The Symbol is a rare drop from Mimics.
The easiest way to get it is to buy 99 Lloyd's Talisman, and find a mimic.
Hit the mimic with your fist to wake it up, then throw a Lloyd's Talisman.
It should sit down. Go up to it and see if you can pick something up (You will be able to pick up the item the mimic normally drops on the first Talsiman), if you don't pick up the Symbol, hit the mimic again to wake it up and throw another Lloyd's. Repeat this until you get it.

I've heard that the patch changed this, and that the last mimic drops the symbol... This is what I mean by conflicting info.

Even if it did, it's faster to just use the Lloyd's.

So far every time I obtained the Symbol of Avarice, it was from killing the last mimic, and every time I killed the last mimic, he dropped the Symbol of Avarice. So... Are the people that didn't get it to drop absolutely sure they killed all mimics? Some of them are pretty easy to miss.
PS: DLC mimics do not count. You can leave those alive if you wish.

I too have gotten the symbol of avarice after killing the last mimic several times (which always manages to be that one in the duke's archives with the crystal knight shield), but I wasn't sure if it was a coincidence. I definitely remember reading somewhere on the internet [citation needed] that patch 1.05 made that a thing. Problem, Renegade?

It has been confirmed that the last mimic drops the SoA. If you passed a few and don't want to go back then just use Lloyd's and your fists. Those are the only ways (other than luck) to get it.
