Two friends and I were in the Catacombs, and both the host and I had crossed the bridge controlled by the lever where Patches stands. Suddenly, as the third guy was crossing the bridge, Patches flipped the switch again and dumped him to his death. When the host went back to the control and spoke to Patches, he "apologized" for accidentally hitting the switch. Said he hoped it didn't give us any trouble (phrased as a question). The host answered yes, and Patches gave him a humanity. Anyone ever had this happen before?

I figured it was well known, only he usually does this to the host or anyone on single player, and then when you respawn he does his apology

I actually had my first encounter with him during NG++, in my previous two playthroughs he never appeared. I always contributed it to the fact I take the tunnels where the crystal lizards are and have to quit and reload to force them to appear.
Oh and when I did encounter him I had dropped behind bridge he flips and was able to walk across it normally and he acted like he had.

I've only seen him once on the lever. I don't know why he isn't always there.

You have to go before Reah appears in Firelink for him to appear in the Catacombs.

Or just don't talk to rhea and her bodyguards when you see them and go to the catacombs right away
he has tried to turn the bridge on me but I think even this got dialed back post 1.05. He is supposed to kill you there. He is very slow in doing it.

im always acrossed the bridge when he does it, bad timing patches....

He is always there as long as Rhea has left for the Catacombs yet and he only flips the bridge if you are slow crossing it. Running over for instance is to quick for him to react.
I noticed this in NG+ where I was going after the blacksmith and was standing on the bridge thinking about something, quite funny actually.
