I keep going back to check, but she's never there. Is this a world tendency thing or what?

Yes, actually there is.
In a pure white world tendency event (you can kill two bosses to achieve this), Miralda will be available in Body Form. She also appears as a black phantom in pure black world tendency. To get PBWT, you can kill her to shift world tendency to neutral, and then die in body form a couple of times. Good luck!

Mild note about this, but Miralda is actually one of the few (if not the only) NPC who won't lower your World Tendency when you kill her in body form. Be at Pure White and kill her and you'll still be at Pure White. To get to Pure Black the easiest way is to suicide in body form 6-8 times. Use a Stone of Ephemeral Eyes, then suicide, then repeat until you're at Pure Black. It may not seem like the best use of your Stones of Ephemeral Eyes, but really, how often are you in Body Form when normally playing anyway?

Really?! I have always killed Miralda, and every time I do that, my world tendency had always been knocked down to neutral. Of course, I was always offline (pretty much a PvE player).
I wonder if being online effect your world tendency regardless?

Seems I was mistaken. She's one of the few body-form NPC's who doesn't shift character tendency :|
Yeah, killing her will lower your World Tendency back to neutral. My bad.
