PvP or PvE?
the basics are the same, equip a weapon in the left hand that has a parry animation, press l2 to parry then if you succesfully parry tap r1 and most weapons will preform a riposte. Some exceptions are whips, bows, crossbows and shields.
In PvE generally you want to parry near the beginning of the enemies attack, the timing is different for different weapons, enemies and attacks however.
In PvP slower larger weapons can be block parried which can be seen in nearly every pvp video. Block the first hit, parry as soon as the animation is over to parry the second hit. You can also blind parry, I do this with some luck, run/roll towards an enemy and parry, chances are they have began mashing r1 and you'll score a parry.

^This, but I just want to give more details:
You can look at the enemy's attack as 2 phases: the wind-up phase, and the strike phase. Errjon is correct, you want to parry "at the beginning of the enemy's attack", but specifically you want to parry at the beginning of the strike phase. So when the enemy draws their weapon back, don't parry; when their weapon is drawn back and they thrust it in your direction to attack you, that's when you want to parry.
You'll know you landed the parry because your opponent will go into an animation: his attack arm will fly back because you repelled it, and his core will be totally exposed. Usually (but not always) this is accompanied by an audio cue signifying you succeeded the parry, and that you can press R1 at that time to land the riposte. You should know that you have a small window to initiate your riposte--it's long enough that you shouldn't miss it if you're not trying anything fancy, but you will miss it if you rolled away, for example. During this window, you can either walk a little closer to the enemy, to ensure the riposte works (they fail sometimes when you're on uneven ground, etc.), or if you're more comfortable with the window you can even swap weapons.
And about PvP, like Err said, it pretty much the same, only there's a good half-second lag. Once you get used to parrying half a second earlier than you would in PvE, you can get pretty consistent at parrying in PvP also. The problem is that most weapons take less than (or not much more than) half a second to execute their wind-up and attack phases, so you can't rely on visual timing anymore. You need to be familiar with every weapon's move-set, know the speed and range of each, and then you'll be able to predict that, if my opponent has begun his strike on his screen, my parry will land... and you'll be surprised how often it works! Most people don't expect to be parried, especially not on their first attack, so as soon as they're within range of you chances are they have in fact begun an attack. So just get familiar with the distance at which you need to be to parry it, and you'll be able to land a parry on their first swing very consistently.

Also, this may seem dumb, but it's worth noting: you can only riposte with R1. Not R2, L1, or L2. So you can only riposte with a weapon in your right hand. So if you wanted to tuck a bandit's knife away in your secondary slot to pull out for ripostes, it must be in your secondary right hand slot.

I have to admit I gave him a little crap in the form of a youtube comment a couple of weeks ago, in response to his backstabbing step-by-step guides. I told him it'd be nice if people would stop teaching noobs to be cheese killers. Then someone pointed out his instruction on how to defend against it and I deleted my comment.
