I've died more times to cliffs than to other people.
In my opinion, the worst cliff in the game it's the giant root that it's in blighttown, the one you have to cross to get the Sealer set. I can go through it easily, but coming back....

The Black Knight in the tower above the Bridge Wyvern. I've never managed to kill him on the first try in any playthrough of mine.

Hey Tolvo, I know this will meet with a howl of disapproval from the purists out there but I found the best way to dispatch the knight at the top of the tower was to sneak up to the very top and cast poison mist while his back is turned. Two doses are required. Cheap it may be but very effective.

Well on NG+ yeah I could do that, but always on regular I won't have a Mist Spell that early. Hmm, I might be able to do something like that with Dung pies though, I know there is the fire exploit where he'll just walk into fires near the boar, but I'm not too fond of that. Funny thing is Havel usually gives me no problem, but that guy kills me all the time haha.

I almost died in the process, as in used all my estus and sitting on less than a fourth of my hp, but I managed to kill him just once on a new game character by attacking him with my sword and shield. Was never able to parry once, or even backstab, he kept doing the R2 attack and if I blocked it I took damage from the shield break almost killing me in a single hit. I don't know if I'll try it again, but holy crap that Black Knight is by far the hardest of them all in my eyes.
I have to say that those titles goes to any enemy you do not know how to effectivly counter. There is such a variety of enemies and with each one having sometimes multiple ways to defeat them, on your very first playthrough even Hollow Warriors can earn all three of those. Overall, I would have to say the whole game is worthy of them. From Software did an excellent job of making the it seem the world is out to get you, even enemies you know how to defeat easily can surprise and kill you, and heck sometimes even the environment is trying to kill you.

The mushrooms in the great hollow give me a ton of trouble as well. In fact, I don't fight them anymore. I always just run past them now. Because I get chain stunned by these little jerks, then as I'm finally getting some ground I turn and see a giant fist smashing through my skull from the parent.

I aggro them all and kill the little ones with a two handed spear then hurral magic at the big ones, backtracking until I reach a root that makes an arch on the mushrooms. I believe it's a pathing error and they just press against it, but they can pass through if they scoot far enough to the left. I can't tell you how much trouble I found myself in when two of them got through, I think for a moment I had 150% move speed

Bonewheels are Satan. The knight in the tower I let chase me to the boar area. Once there his attacks are easy to avoid

Oh yeah, I've gotten killed a few times by being careless. I go to the outcropping the Black Knight is on which instantly aggros 3 of them and then I use my Dragonslayer Bow to kill them all, I think Wrath of Gods can reach them if you go to the lower tier, but I've never tried it really.

These days I just run past everything haha, I drop near to Pinwheel then just run in between the bonewheels and go through the fog gate haha.

Yeah, I tried that and actually got killed a couple times, I'm not very good at running from a fight :| . So now whenever I venture down that way I kill them to sate my rage.

The most deadly enemies in the game are all the BB glitch abusers who invade right before the bell tower gargoyles. If you've ever tried to kill people with +5 lightning or +10 fire weapons with your starting/low level gear, it's near impossible... I don't think I've ever seen anyone invade there who wasn't obviously abusing the glitch...

The NPC Bandit in the Forest, I swear he's more talented than most of the people who invade me.
Example:

I think most people have covered my choices in terms of 'tough' enemies. In terms of annoying though, the toxic raspberry men in the painted world get a special mention. And the harpies come to think of it with their 'imma sit on yo face and stab yo in the chest for half an hour' attack.
