It's still hard to get it to go, and it seems to require hits from more than one person, so its pvp applications are still quite limited. Nonetheless, I managed to use semi-effectively in the forest in conjunction with the gargoyle shield. I say "semi-effective," because I probably would have been better off simply two handing my weapon and casting inner strength rather than trying to bait people into setting off my karmic justice.

Does it have to be a shield? Obviously my first non-100% shield would be grass crest, but blocking with a claymore is less than 100% too.. have you tried that yet?

I'd assume as long as you're taking repeated hits that do some damage it would go off.

No, I haven't tried a claymore, but I see know reason why it shouldn't work. Grass crest is a fine choice. I chose gargoyle because it has high elemental resistance and it seems to block a lot of bleed, which is important if you turtle.

Yes, repeated hits to the shield from multiple toons (forest campers -- gotta love 'em). However, I couldn't seem to get it to pop against one person even when they were wailing on me repeatedly with an uchi.

Oh, and to save everyone the trouble, it doesn't get activated by the self-damage inflicted by the chaos blade. I tried that out, hoping that I would discover an awesome use for three otherwise crappy items (KJ, garg shield, and chaose blade), but no luck.
