    4 play throughs and i can't get the damn thing to fall off.

    have some slabs, fully upgraded Channeler's Trident and titanite Catch Pole, both ghost blades...

    I had a lot of trouble with this too, I found that using the far ends and waiting for him to come to you, then running towards him so he begins an attack animation, then running behind him and preforming a running attack on the tip of his tail was the best way for me to do it. I died plenty of times attacking the base/middle of his tail, doing my best not to kill him.
