I know that sounds like a noob question, but what is the exact definition of it? Is the lock-on BS considered a lagstab?

A lag stab is where they BS you, but aren't behind you. There is lag, so what you are seeing is not what they are seeing. You may be lagging 1 second behind them. So, they appear to teleport to you and BS you, or BS you when they look like they are in front of you. Even worse, some people have a lag switch. They will purposely cause lag before they BS you.
Pivot BS is when they run past you, pivot rapidly, and BS you. That is a good technique. However, it can be abused. They may be poise-gouged so you can't interrupt their charge and they will run through your attacks (aka poise stab). Or, they will constantly strafe and fish for BS. Both of those techniques are boring and annoying.

When you're rolling and then sundently you appear being backstabbed 1 mile away from the guy.

... and when you strafe away from the guy while he's running backwards to you, and you know that guy is really behind you already, or more specifically: you are actually running backwards to him in his world... when this happens I always lock off, turn around and hit the air behind me. I always manage to do damage to the guy in such cases, but still I often miscalculate sooner or later and get lagstabbed anyway...

I finally found a video of someone experiencing the same "worst lagstab ever" that I did, in the forest on the bridge. Mine was arguably even worse because I was low encumbrance and running, and somehow had this done to me by a lumbering havels guy:
