--No clue about this one. Maybe it was supposed to say this when you find Dusk after defeating Manus?

--I know some people attempted a 'no bonfires' run, where they also tried to extinguish all the possible pre-lit bonfires by killing the nearby firekeepers. I wonder if perhaps there was originally supposed to be some sort of secret/ easter egg/ reward for doing this, and the option might've existed for the only bonfire you can't put out - the Stone Dragon bonfire. Or maybe it was an annoying PvP feature that was cut. Put out someone's bonfire, and when they respawn they wind up back at the previous bonfire they used instead...
Seal of the Great Lord was broken...
--I'm assuming that this would indicate the disappearance of the yellow fog gates after placing the lordvessel, but the cutscene shows us that already.

--This is almost certainly from Oscar's cut storyline. There is dialog where he asks you if you want to team up in the Darkroot Forest, which clearly offers the player a Yes / No option. This is likely the yes / no prompt for the player.

Well, how long has it been? Glad to see you safe. You must be the same as I; in search of the grave of Sir Artorias. But, be careful. This forest is the territory of a fierce band of thieves. They assault any and all who seek the graves…. What if we were to join forces? How about that? If we worked together, we may escape the bandits. Does that appeal to you?
