I once met with a fate that (in character) may have been worse than death. In Demons' Souls, I once fell off the bridges in the second zone of Latria. The game glitched and I fell into an infinite white abyss. I quit, reloaded and found myself back on the bridge.
I played Ocarina of Time (in the Nintendo 64) for hours. I had beaten two or three dungeons, killed several of the fleeing poes in Hyrule field and I had not saved yet. I was on Epona and jumped over and fence and I changed direction to a diagonal angle in midair-and I landed on the fence. I was unable to dismount, Epona was only able to walk in place and I couldn't pause-meaning I had to turn the game off without saving.
In a sci-fi-replaced-by-magic dungeons and dragons campaign, my character and most of the rest of the party were entering a planet and parachuting out of the spacecraft. My character had a problem with being a daredevil and decided to wait to open his parachute until the last moment. The DM had me roll an intelligence check to accurately gauge the distance. With ten being average (for a human), I came up with a total of 7. The DM had us roll to see if our parachute's worked. Only mine failed. The DM had me roll a luck roll to survive by some miracle. I rolled a natural 1. (the worst roll possible; that's an auto-fail, or in some groups, as a house rule, including ours, a critical failure) My character turned into a fine red mist as he crashed down the red maple forest.
