Getting caught with the R2 slamming attack of a big weapon, ie the Zweihander or one of the clubs, out of mid air is pretty brutal. Just happened to me about an hour ago. I was trying to land a jumping attack and a guy splatted me right out of the air with the Grant, like Mr. Miyagi catching flies.

Rung the second bell, went down to cease ceaseless discharges shenanigans, then headed all the back up to head to Sens Fortress. At the Undead Church I aggro the three Balder Knights just for fun, and like a very brave idiot I try to parry when all three are surrounding me. I miss the parry and in about 0.02 seconds "YOU DIED". No big deal, it happens, but wait....

Fighting the Stray Demon in the Asylum, using Lifehunt Scythe. Forgot that this weapon causes you to bleed too. Demon died, I died AT THE EXACT SAME TIME! NO SLAB!!!!! This still haunts me to this day. :affraid:

Last night I died fighting Kalameet with around 40,000 souls on me, and I hadn't rested at the township bonfire, so I had to run back through the Royal Woods. I was really paranoid about getting cornered by Stone Guardians, so I skirted the right side of the area and was super careful not to aggro anything, only to turn the corner and fall down the empty elevator shaft. Seems like the only time I ever do crap like this is when Im being overly careful on my way to pick up dropped souls

How about running back to get 25k's worth of souls and 39 Active Humanity that I had just lost to a Guardian in the Royal Wood, only to turn right at the end of the bridge and run right off the edge!
Or getting Supersized Smough down to a final blow using only a pair of +5 Caestus's, only for the fat tub of lard to butt slam me into oblivion!
Or even better still having successfully taken the 3 in a 3v1 down to a 1 in the Darkroot Garden, only for me to barrel roll off the edge to avoid a passing blow that would've only taken away a fraction of my HP's when the Host was down to being barely one hit away from death!
(That one still haunts me to this day! )
I could go on, but I think I've shamed myself enough for now!

I was on my cursed run and fighting O&S and got to the upgraded Ornstein. I was trying to kill him with a war-pick and got **** with the last quarter of his health. I missed the dodge and he impaled me and I died automatically.
Also on NG+4 I was in the Tomb of the Giants and fighting Nito. Unfortunately I got one of the large skeletons from the back to kill me. I didn't think it was a big deal until I realized the last bonfire I actually rested at was at the Duke's Archives.

so i drop my white soap stone at the entrance to Sen's and proceed up through the house of traps. i was just about to finish off the first manserpent with the flamberge swords when i got the message that i'm being summoned. sweetness!
i help the host all the way through to the Iron Golem and we kick its a**. Tarkus did most of the work, but whatever, he's awesome. easy money.
i start spawning back in my own world when it dawns on me that i am spawning right under a swinging guillotine! sweet baby jesus have mercy!
so i land in Sen's basement and by some miracle i survived the fall. Praise the Mother F***ing Sun!
but my joy didn't last very long. why? because of the titanite demon catch pole i just took to the face. that's why.

Getting my poor knight beaten up and impailed by the Blue creatures in Seaths prison, then thrown over the railing and falling to my death. btw I too am collecting tales of death in a thread here .
